For families accepted to TSNS for the 2017-2018 school year:
The following forms are required to complete enrollment.
Please complete these forms and submit them to the Nursery School Office by no later than July 19th, 2017.

2. Required Forms From Health and Dental Providers – due no later than August 1st, 2017.
3. Additional Required Forms – please print, complete, and return to the Nursery School Office by no later than July 19th, 2017
4. Special Forms (available if medically necessary)
a. Allergy Care Plan (if your child has a known allergy)
b. Food Allergy Posting Information (if your child has a known allergy)
c. Permission to Administer Information (for any child receiving medicine while at school – requires doctor’s signature)
